Sverige går idag mot en mer patientfokuserad vård. Personal Health Records (PHR) är en tjänst som möjliggör för patienten att samla och distribuera sin egen medicinska journal. Trots de patientfokuserade egenskaperna som PHR besitter är vetskapen om PHR liten i Sverige.Med syfte att undersöka PHR har vi utfört en studie som visar om PHR har en potential i Sverige. Nitton intervjuer utfördes varav sjutton stycken med diabetespatienter, en intervju med en representant för läkemedelsindustrin samt en intervju med en representant för sjukvården. Dessutom utfördes en patientenkät med diabetiker.Studien visade att PHR har en potential i Sverige, framförallt bland den viktigaste intressentgruppen patienterna fanns ett visst behov och upplevdes nyttan som stor.

The County Administrative Board is an authority with big variations in their work. The fact that the County Administrative Board work with so many different issues makes it difficult for the general public to get an insight in what they do and what kind of archival material they create. This handbook is made to illustrate what the County Administrative Board do and how you can find your way to the archives and its documents, both as an employ and as a public visitor.The Handbook begins with a historical overview of the County Administrative Board, from the 17th century to today, with focus on the County Administrative Board of Uppsala. As the Handbook is made for both employees and for the general public several ways to enter the archives are described.Since the employees at the County Administrative Board of Uppsala have access to the archives them self the Handbook also provides detailed descriptions about how to find the way to the records. These descriptions both explains the way to archives organized in dossier and archives organized in chronological order, both provided at County Administrative Board of Uppsala.All the records that the County Administrative Board of Uppsala has generated though the years are not available in the building of the County Administrative Board but at The National Archive in Uppsala.

Background: Insufficient nutritional support is associated with prolonged hospitalisation, impaired wound healing and impaired survival for patients in intensive care. In severe sepsis and septic chock, calculation of nutritional need is complicated since the metabolism is affected by decease. Aim: The aim of the study was to investigate nutritional support and to examine the quality of nursing documentation of nutritional status and nutritional support in patient records in severe sepsis and septic chock.  Method: The study was conducted as a retrospective investigation where 64 patient records were studied. The quality of documentation was examined in 10 patient records using an examinational model.Findings: Calculation of average nutritional support showed insufficient supply particularly in the two first days of intensive care. During the next five days nutritional supply was higher but individual variation was seen, why a clear picture of nutritional support is hard to detect.

For my advanced degree in archival science, I have organized and catalogued the Blomqvist family archive.  The Blomqvist family archive consists of records from the end of the 19th century to the beginning of the 21th century. The records in the archive depict and describe different aspect of the Blomqvist familys life and work. Specific problems and theoretical questions that appeared during the cataloguing of the Blomqvist archive were related to the great presence of objects and photographs in the archive. A discussion concerning the role of objects in archives and within archive institution took place. The problem with cataloguing photographs as well as the problem of searching for photographs in archives, were also discussed.       .

The Swedish geographer Gerd Enequist (1903-1989) left behind great amounts of private and work material when she passed away. This was donated to the University Library of Uppsala, Carolina Rediviva, in three different accessions. This resulted in three different record. The subject of this report is the progress of assembling these record into one complete record.The registering led to many important decisions such as the role of provenance and the relation of private and public documents. Above all the special nature of personal archives have been a topic of discussion.

Introduction: Balance between the essential nutritive substances is important for aperson to maintain health. The surgical patient can be stricken with conditions, whichhave a negative impact on this balance and can lead to malnutrition. Working as a nurseit is important to identify patients that are malnourished or is in the risk zone ofmalnourishment since 30 % of all patients in hospitals are malnourished.Aim: The aim of this study was to examine the compliance to the guidelines fornutritional assessment and parenteral nutrition on a surgical ward. Also whether thenutritional treatment/-support was reported to the next caregivers if the patient wasmalnourished or was in the risk zone of malnourishment at the time of discharge.Method: 80 patients enrolled on the ward in the beginning of year 2011 were included.Audit of patient records was made according to the hospital guidelines for nutrition andparenteral nutrition and followed a study specific protocol.Results: The examination of the Case records showed that the guidelines for nutritionwere partly complied with. Nutritional treatment/-support was reported to the nextcaregivers when needed.

This thesis is about a potential trade-off between integrity and long-term usability in the choice of file formats for preservation of text documents. Five common formats are discussed: plain text, PDF/A, Office Open XML Document, Open Document Text, and Markdown. The formats are compared with respect to four criteria related to integrity and usability and to the records continuum model: support by widely used software, stability, rendering of contents and reusability.It is concluded that no single format is optimal with respect to all four criteria, when it comes to preserving typical documents in a modern environment, with more or less complex formatting and document structure. Therefore, the feasiblity of using two or more formats for preservation of a single document (e.g. PDF/A combined with Markdown and/or Office Open XML) is discussed.

This thesis aims at exploring a national collaborative gateway-service, PictureAustralia, hosted at the National Library of Australia. This will be done by means of investigating some underlying principles for descriptive standards and conventions as of the library discourse which is the objective of bibliographic control and access. The reason to do this is to bring some main features of these principles into the context of a cross-institutional cooperation between libraries, galleries/museums and archives in a picture database service. The attention will lay on the objectives of what functions the system is designed to perform to give access, as defined by Elaine Svenonius in the Intellectual Foundations of Information Organization 2000, and how these objectives will affect this particular database-service collocating performance, which is a system- and logical approach. Though, this study will be done in an explorative and qualitative way, by a case-study, which is to examine some sample metadata records emanated from different institutions in this project.

For centuries it was a crime to be without work or an income in Sweden. Those breaching this law were called vagrants and could be sentenced to forced labor. They stood outside of society in many ways as they were being criminalized. Society's view of them becomes clear by the laws enacted against vagrancy. The investigations that were made in creating the laws do not only say something about society's view of vagrancy, but something about the society itself.In Sweden we have a unique collection of investigations and referrals that are public records through Freedom of the Press.

This examination paper is the account for the author?s work while arranging and registrating the business archives and business records of the Herman Bergman?s Foundry in Stockholm, Sweden. It is part of the one year Masters program in Archival Science at the University of Uppsala. The records are from the beginning of the 1900s to the 1970s. Most of the business archives are from he 1940s to the 1950s.

This project practise methods used in the interaction design discipline to develop a concept related to the Swedish Guinness World Records Magazine. This project is a challenge to accommodate both the users? and client?s needs. We focus on interaction and the experience, which is acquired through the use itself, and how to connect it to a theme from a client. The aim of this project is to create a meeting place for children where they can interact with technology.

The aim of this thesis is to investigate how the indexing of fiction for adults looks like in the public libraries that don?t buy indexed records for fiction from Bibliotekstjänst (BTJ). The questions that I want these libraries to answer is: - Public libraries that index fiction for adults themselves: 1. What tools do they use when indexing?, 2.

This is a study that will focus on the possibility to learn information about a town and its inhabitants of the 18th century by analyzing individual titles in the church records. The records that are used in this study are marriage registration administrated by the local church of the town of Uddevalla. The records are very detailed and each individual registered have a title, such as ?tradesman? or ?maiden?. Since the source are a registration of all the weddings that took place during the period of 1700 to 1749, the source gives very broad and valid information about the inhabitants of the town.

AbstractThe aim of this study was to examine to what extent documentation of pain assessment with Visual Analogue Scale (VAS) on patients with pain problems was carried out. The study also explored how the documentation followed the existing guidelines of pain. The material was analyzed with descriptive statistics and a qualitative content analysis. The results shows that 43,24 % of total 37 analyzed patient records contained documentation of pain with VAS on the day of hospitalization. All studied journals contained documentation of pain.

The main purpose of this qualitative study was to describe clients? descriptions of the professional role case manager and how it relates to the job description provided from a project in which both case managers and clients were participating. Both what case managers do and are expected to do were explored. The project involved case management in the purpose to strengthen and enhance the care and quality of life for clients who are dually diagnosed. Interviews were conducted with five clients and the result was analyzed with theoretical concepts from role theory.
