Ett stort antal människor dör på grund av skador som de fått inom vården och som hade kunnat förebyggas. Det finns siffror som pekar på att dessa vårdskador överskrider antalet döda i trafiken. Vid Östergötlands landsting finns sedan år 2005 en fast enhet som arbetar med patientsäkerhet. En av deras uppgifter är att, tillsammans med berörd verksamhet, utföra händelseanalyser inför anmälningar av Lex Maria-fall. Den analysmetod som används av patientsäkerhetsenheten är uppbyggd på en epidemiologisk olycksmodell.Syftet med denna studie har varit att använda analysmetoden FRAM (Functional Resonance Accident Model), som är en systemisk analysmetod utarbetad av Erik Hollnagel, i en vårdrelaterad kontext, någonting som inte har gjorts tidigare.

The fallout from the Chernobyl nuclear accident resulted in large deposits of caesium, iodineand strontium as well as noble gases in various parts of Sweden. 137Cs has a radioactive halflifeof about 30 years and is therefore one of few radio-fission products remaining inmeasurable quantities in the Swedish soil. Radiocaesium behaves similarly to potassium insoil-plant systems and is easily absorbed by plants. It is important to understand how 137Csbehaves in different ecosystems in order to minimize the risk toward humans.The aim of this study was to determine how potassium fertilization influences radiocaesiumcontent in different forest plants and fungi. Potassium fertilizer was spread once in 1992 withapproximately 200 kg KCl per ha.

This investigation is aimed to analyze and gain insight into how young people of the "second generation immigrants" identify themselves in a society where they are seen as immigrants of the environment in view of the name. Also, how they identify themselves during trips to their home/country of origin are of relevance to see if they are affected in their own identification, depending on how they believe they are seen from the outside.Through a survey, the statistics were produced, which serves as a preliminary to the larger part of the investigation where six individuals were interviewed in order to give deeper answers to the questions sought. The answers that emerged were then analyzed in relation to selected theories.In connection with the investigation it has emerged that the young people in question often identify themselves in such a way that they think that the surroundings look at them. It is in relation to how others see them as they see themselves, which creates a distance between the surroundings and themselves. The aim of this study is to describe how reviews of literature in Bonniers litterära magasin and Expressen changed during the period 1964-2004. The investigation is based on 20 reviews of novels and short stories, which are analysed with a method described in Hellspong and Ledin (1997) and inspired by Halliday. The theoretical frame of the investigation is founded on critical discourse analysis (Fairclough 1992 & 2001) and Habermas´social theory of the public sphere.The investigation shows, among other things, that the reviews became shorter and easier to read in both paper during the period. The investigation also shows that the genre´s style, composition and contents are relatively stable, and that current ideologies in society have a tendency to be reflected in the reviewers´evaluation of the books. To generalize, the reviews thus developed from a poetic genre in 1964, to become political in the 1970s, academic in the 1980s, feminist in the 1990s and finally an intermedial genre in 2004..

When an animal is approaching the road, it is registered by sensors, and warning lights along the road are lit to inform drivers of the potential danger. The aim of this study is to describe ideas put forward by Sweden Democrats (Swedish: Sverigedemokraterna) about Christianity in Sweden and the Church of Sweden as a Folk Church, a church for a whole people, and how the political party uses Christianity as a part of constructing a Swedish identity. The investigation is based on official documents, newspaper articles, statements and political motions which are analysed using a form of text analysis called idea analysis. The theoretical frame of the investigation is founded on Claire Mitchell?s theory about religious content in ethnic identities and Einar Billing?s and Johan Alfred Eklund?s thoughts about what makes the Church of Sweden a Folk Church.The investigation shows, among other things, that the Sweden Democrats? view of the Church of Sweden is similar to the thoughts of Eklund but they differ in their lack of references to theology. The investigation also shows that Christianity serves as an important marker for the Sweden Democrats in creating a Swedish identity. .

The purpose of this paper is to explore what objects that can be seized and how the management of a seizure should be conducted. Ability to take enforcement against an individual should be carefully weighed against the rights and freedoms that exist. Seizure is one of the various restraints that exist, and the regulation on which they appear is mainly in the Code of Judicial Procedure, Chapter 27th. To be able to apply coercive measures, it is fundamental prerequisite that a criminal investigation is initiated, but there exist some significant exceptions to this. There are also rules about who has the right to provide for the seizure, the decision can be taken by a prosecutor, an investigator, a police officer or, in special cases, the court.

Water is the most essential resource for life. In cases where drinking water is processed from surface water it is important to ensure the raw water is of good quality, and is suitable for processing through the water treatment plant.The drinking water for Västerås is sourced from Västeråsfjärden, in the western side of Lake Mälaren. The city itself is home to marinas, ports, a wastewater treatment plant, the Svartån outlet, sewage pumping stations and stormwater outlets. Because of the negative effects these activities have on surface water quality, Mälarenergi AB is interested in relocating the raw water intake point to Granfjärden, 10 km east of Västeråsfjärden. At Granfjärden the intake point would be less exposed to the activities in Västerås, and could be placed at a greater depth, resulting in better water quality.In this Master?s thesis the water quality and the pollution risks at Västeråsfjärden and Granfjärden sites are compared.

The purpose with this study is to examine how the media coverage of the train accidentoutside Kimstad, on the night of September 12th 2010, was framed in one local (Norrköpingsnyheter) and one national newspaper (Expressen) and how the coverage of the accident wasdifferent in the local newspaper compared to the national newspaper.The material is 12 articles within a fixed time limit which is from September 13th toSeptember 15th. The material consists of six articles from each newspaper.The method we use a qualitative text analysis with its basis in medialogic, news value andframing theory.In the essay?s analysis we show a synthesis of both newspapers coverage of the train accidentoutside Kimstad and how they stand in relation to the theory of news value and we end with aterminative comment about the results.The conclusion shows that the coverage of the event was different in many ways. Expressenuses a more dramatic framing and a more sensational language which describes the eventmore badly than it was. NT uses a more informative and objective information which almosttrifles the event but which informs about relevant information for the locals.

The purpose of this study is to examine how adult students, studying at an upper secondary school for adults, search for information for their independent tasks in different subjects. These independent tasks are frequent in most subjects and are a part of the ?investigation-work?. For most adult students this is a new way of learning. Fifteen qualitative interviews with adult students have been conducted.

I am looking at this resource from a gender point of view. I want to see if ELT has thought of this perspective. Are there as many websites for girls as there are for boys? I want to see if the text is separated between gender.

We have decided to conduct a case study of German and Swedish mid-level managers within the automobile industry. Primarily we used a qualitative method of investigation and a phenomenographical methodological approach in order to conduct an in depth analysis. We conducted semi-structured interviews with six mid-level managers each in Germany and Sweden. The semi-structured interviews allowed us to conduct flexible interviews which enabled these managers to speak relatively freely about their own view of leadership. Throughout our investigation we have based our results on the empirical findings and at the same time sought explanations to different phenomenon in relevant secondary data, such as management literature, thus used the abductive method of investigation..

The chief aim of this study is to investigate the impact of context on written peer response. A second aim is to explore students? attitudes towards peer response. The investigation has taken place at an upper secondary school in the south-east of Sweden, involving 20 students in grade 2 and 3 in the course Swedish C. This is the first investigation of the activity with feature film (video) in Swedish publiclibraries.Due to the Swedish copyright legislation, the activity with feature film in Swedish publiclibraries was not possible until an agreement was set between Kommunförbundet and SverigesVideodistributörers Förening in 1989.My investigation is based upon a form that I made in cooperation with Statens kulturråd.The form was sent to the main public libraries in the 286 municipals in Sweden. 250 (87 percent) of the public libraries answered the questions. 130 (52 per cent) of the libraries whoanswered had an activity with feature film.The main reason the libraries bought feature film, was that they hoped that feature filmwould attract new visitors to the library.
