Säker läkemedelshantering - Faktorer som påverkar sjuksköterskan
Läkemedelshanteringen är ett av sjuksköterskans ansvarsområden inom hälso- och sjukvården. Utifrån kunskap och lagar skall den bedrivas på ett patientsäkert sätt men trots det sker läkemedelshanteringsfel vilket innebär risker i patientsäkerheten.Syftet med studien var att beskriva faktorer som påverkar säkerheten i samband med sjuksköterskans läkemedelshantering. Studien genomfördes som en litteraturstudie. Resultatet visade att vanliga läkemedelshanteringsfel är utebliven dos, fel administreringstid, fel dos, fel administreringssätt, fel läkemedel och läkemedel till fel patient. Faktorer som påverkar sjuksköterskans läkemedelshantering är felaktiga och dåligt skrivna läkemedelsordinationer, distraktion under läkemedelshanteringen, bristande rutiner samt införande av ny teknologi.
Säkerhetsklassificering av dammar : En kartläggning av system i Sverige, Norge, Finland, Schweiz, Kanada och USA
In a comparing survey this thesis investigatesclassification systems for dams in Sweden, Norway,Finland, Switzerland, Canada and USA. Theinvestigation is aiming at an understanding of howpotential consequences of a dam failure are takeninto account when classifying dams. Furthermore,the significance of the classification, regarding therequirements on the dam owner and surveillanceauthorities concerning dam safety is considered andreviewed. The thesis is pointing out similarities anddifferences in the line of dam safety.The survey of dam safety is firstly based upon legalrequirements in the different countries andsecondly on guidelines regarding dam safety. Damowners generally are ultimately responsible for thesafety of their dam, and often have their ownroutines regarding how this safety is to be achieved.In this thesis, however, the legal requirements aremainly focused upon.The investigation has resulted in an outlining ofeight areas, where the classification systems in thevarious countries have been compared.

AbstractThe Swedish school is the biggest place of work in the country. Statistics about the Swedish school shows that both students and teachers work in an environment who has elements of violations, threats and violence, and study environments where many students do not feel they have a study environment during class. The purpose with this study has been to investigate if teachers and principals is given the right conditions in their respective educations to pursue their professional profession to achieve the objectives of the goals with the systematic safety and work environment of the school's safety concerns. The study has been designed as an investigation about respective educations syllabus and has been examined by the presence about systematic safety and knowledge about work environment law in respective courses. The examination about respective educations syllabus has been done by a qualitative content analysis.
Psykosocial arbetsmiljö och säkerhetsklimat: en explorativ studie på mindre flygplatser i Sverige
The aim of this paper is to examine the psychosocial work environment and safety culture in minor Swedish airports. The purpose is also to investigate the relationships between these two. The paper deals with theories regarding psychosocial work environment, organizational culture and safety as well as safety culture and safety climate. The participants are air traffic controllers working in minor airports in Sweden. The psychosocial work environment was measured with Copenhagen psychosocial questionnaire (COPSOQ) and the safety culture with the safety culture assessment questionnaire (SCAQ).
Införande av IT-system i slutenvården : En studie vid Lasarettet i Enköping
As the ability to computerize processes and tasks increase, the proliferation of complex systems in our society continues. How these systems are deployed in the organizations where they are meant to be utilized has been shown to have a large impact on how well they function.In January of 2011 the Enköping hospital initiated the deployment of an IT-support for drug management, a module in the general health care system Cambio COSMIC. Other health care providers in the county, and in the rest of Sweden, have had mixed feelings toward the module. Some have outright refused the module, claiming that it is detrimental to Patient safety. One of the arguments for deploying the module in Enköping, however, was that it was needed to improve Patient safety.The Enköping hospital is considered to be a well organized institution with good leadership and earlier good experiences regarding IT-systems.
Skyddande isolering i samband med stamcellstransplantation : Patienters upplevelser av den skyddande isoleringen samt uppfattningar om sjuksköterskans roll under vårdtiden
When going through a stem cell transplantation a patient is forced to undergo protective isolation to inhibit risk of opportunistic infections related to the low leucocyte count as a result of aggressive pre-transplant chemotherapy treatment. Protective isolation has shown to have an impact on patient experiences during treatment and the aim of this study is to elucidate experiences of protective isolation of patients undergoing hematopoietic stem cell transplantation and to deduce patient perceptions of the role of nurses in protective isolation care. The presented study was performed as a mixed qualitative and quantitative survey at a hematopoietic ward in Sweden. Results were analyzed by Mayring (2000) and by descriptive frequency charts and show that patients in protective isolation experience alienation, necessity, safety and anxiety with regards to their protective situation. Considering the role of nurses patients perceive their role to be attendant, informant and acting as an executioner of medical implementations.
Barnsäkerhet på förskolan : en kvalitativ jämförelsestudie
The purpose of the present study has been to investigate the attitudes of personell involved in the preschoolorganisation in a Swedish commune regarding injury risks for children in pre-school. Sixrespondents were interviewed in a qualitative interview study.Two different pre-school units were investigated, and it was found that they had surprisingly largedifferences in how they were working with children safety. However, in summary it can be said that therespondents perceived that child safety should encompass a safe and secure environment, however withthe important caveat that accidents can never be completely excluded, due to the natural anddevelopmentally important play of children.The investigated commune and pre-school units improved children safety mainly by using yearlyplanning meetings (Sw. "barnskyddsrond") where the physical environment in the school wasinvestigated together with external specialists. However, there are important differences in how the unitsworked within this framework, some units actively encouraging and others discouraging children andparent participation in the work..
Upplevelse av kommunikation, samarbete och säkerhet hos sköterskor som arbetar på en operationssal.
Syftet med studien var att undersöka hur sköterskor som arbetar på en operationsavdelning där WHO´s Surgical Safety Checklist används, upplever kommunikation, samarbete och patientsäkerhet.Kort metodbeskrivning: Kvantitativ metod. Design: Deskriptiv, retrospektiv, icke-experimentell tvärsnittsstudie. Datainsamlingen genomfördes på en operationsavdelning som använt WHO´s Surgical Safety Checklist drygt ett år. Deltagarna fick anonymt besvara ett frågeformulär med strukturerade frågor, en översatt version av the Safety Attitudes Questionnaire (SAQ) anpassad till operationspersonal (OR). I studien undersöktes 3 av frågeformulärets 6 teman: säkerhetskultur, samarbetsklimat och stressidentifiering.
Seeking Safety som behandling vid PTSD : En single case studie i psykiatrisk öppenvård
Denna single case studie, med multipel baslinjemätning över beteenden, utvärderade huruvida frekvensen traumarelaterade undvikandebeteenden minskade vid 12 sessioners behandling under 6 veckor med Seeking Safety (en behandlingsmanual med copingstrategier för PTSD och samtidigt missbruk), hos patienter med PTSD som inte bedömts lämpliga/redo att direkt börja traumafokuserad KBT. Därutöver utvärderades huruvida behandling med Seeking Safety, kunde öka villigheten att genomgå traumafokuserad KBT. 4 patienter deltog i studien varav 3 fullföljde behandlingen. Resultaten visade en svag tendens i riktning med hypoteserna, men studiens resultat i sin helhet ger ej stöd för behandlingens effekt hos patientgruppen. Av denna studie går ej att utläsa huruvida behandling med Seeking Safety kan leda till minskning av traumarelaterade undvikande-beteenden..

The aim of this study is todescribe the safety culture at a specific production unitand research what work which has to be done in orderto change and improve the safety culture at AvestaWork.There are several opportunities for improvementpointed out in this paper and here are some mainresults presented. First and foremost, the concern ofsafety among blue-collar is often secondary due toproduction.
HACCP-Implementering och tillsyn
The food-safety legislation in Sweden were changed and updated in 1996, much because of demands from the European Union for a common legislation regarding the food-safety issues in the EU. This report discusses how the Swedish authorities are dealing with the new legislation in Sweden. Further on the report presents how the food safety officers in the municipalities in Sweden are implementing this new legislation in their daily work, especially the requirements regarding the HACCP, Hazard Analysis Control and Checkpoints. The report shows that the implementation is quite slow and that many foodsafety-officers don?t implement the legislation in the same way all over the country.

Det är väl känt att sjuksköterskor ofta har en hög nivå av arbetsrelaterad stress. Syftet meddenna studie var att hitta orsaker till stress hos sjuksköterskor och hur detta påverkaromvårdnaden av patienter. Studien genomfördes som en litteraturstudie vilken omfattade 12artiklar med varierande metoder och upplägg utförda i Europa och Nordamerika. Orsaker tillstress identifierades och tematiserades i två teman; Hur påverkas patienten ochomvårdnaden samt Hur påverkas sjuksköterskan. Följande huvudsakliga resultat hittades:Hög bemanningsnivå visade samband med hög omvårdnadskvalitet, hög arbetstillfredsställelseoch låg stress.

This essay aims to examine how safety is reflected in local plans. This was done by describing the physical environments, which are addressed in the local plans and its impact on safety. It has also been undertaken to see if certain physical environments with respect to safety have been addressed differently.The paper is made with the qualitative method Thematic analysis. The method is used to analyze the three local plans that have been available during the essay. The analysis was done with my created themes Traffic separation, Accessibility, High vision, Light and bright and Human presence.The results have shown that the local plans takes up physical environments that are good for safety.
Sjuksköterskans roll i palliativ vård : en litteraturstudie
Caring for the suffering, dying patients and giving support to relatives is probably one of the toughest jobs a nurse can encounter. Palliative caring is, with different measures, the prolonging of life whereas hospice caring is giving patients a meaningful and natural journey toward death. The hospice philosophy is grounded on comfort, communication and peace. The expectation is that patients will have a dignified, peaceful death. The purpose of this study was to describe the nurse's role in palliative care.
