Att vårdas på sjukhus : Patienters upplevelser av vårdmiljön på sjukhus
Bakgrund: Vårdmiljön på sjukhus har traditionellt utformats av arkitekter. Fokus på patienten har skiftat från subjekt till objekt, för att åter gå mot en syn som subjekt. Intresset för utformning av vårdmiljön har de senaste åren ökat. Problem: Patienterna inneliggande på sjukhus befinner sig i en utsatt situation. Sjuksköterskor på sjukhus har lite kunskap om hur vårdmiljön påverkar patienterna vilket behövs för att kunna arbeta efter ett personcentrerat perspektiv.
När det inte blev som planerat : Patienters delaktighet i det infektionsförebyggande arbetet och i upptäckten av tidig postoperativ infektion vid höft- eller knäprotesoperation
Höft- och knäprotesoperationer är idag en vanlig operation vid artros i höft- och knäled. Trotsminutiösa infektionsförebyggande förberedelser inför operationen drabbas ändå en delpatienter av postoperativa infektioner. Vid ortopedisk proteskirurgi är postoperativainfektioner ett direkt hot mot den nya inopererade leden och kan leda till långabehandlingstider som påverkar patientens livskvalitet under lång tid.Syftet med studien var att utforska patienters möjlighet till delaktighet i tidig upptäckt avpostoperativ infektion utifrån given information vid operation för höft- eller knäprotes.Semi-strukturerade intervjuer genomfördes med tio patienter med diagnostiserad postoperativinfektion efter höft- eller knäprotesoperation. Transkriberade intervjuer analyserades medinnehållsanalys. Journaler granskades för att beskriva patientens tidigare sjukdomar.Resultatet visade att första tecknen på infektion kan beskrivas med temat: Vad är normalt ochvad är inte normalt.

Several studies show that the clinical reality in psychiatric care does not correspond to the demand of consumer participation from a variety of policy documents that has been produced recently. Studies also show that there is a lot to do in improving, patients with sever mental illness, becoming more involved in their own care. The nurse has a central role in this work. The Norwegian nursing theorist Jan-Ka?re Hummelvolls holistic, existentialistic model for psychiatric nursing were used as theoretical ground.The aim of this study was to examine how nurses in psychiatric outpatient units, caring for patients with psychotic disorders, experiences consumer participation.Qualitative phenomenological method was used.

Background: Patients who deliberate self harm often feel disappointed with the health care. The consequence of this may be that the patient avoid to seek help after self harming. Nurses' often experience these patients to be difficult and hard to deal with. Both patients' and nurses' thoughts about the situation may affect the situation in a negative way. A good relationship between the caregiver and patient is important.

Background: Studies showed differences in attitudes towards patients with mental illness in nurses who worked in somatic care compared to nurses who worked in psychiatric care. The nurses in somatic care stated more negative attitudes to mental illness in relation to the nurses in psychiatric care. Studies also showed that work experience affected attitudes towards mental illness. Nurses who often came in contact with people with mental illness had less negative attitudes and prejudices against mental illness. Aim: The aim was to examine the experiences from nurses in somatic care, caring for patients who also have a psychiatric diagnosis.

Early extubation of cardiac surgery patients has become increasingly important. The assessment of the patient before an early extubation is crucial and the intensive care nurses (ICU nurses) in this estimation is there for very important.The aim of this study was to examine critical care nurses' knowledge of early extubation, and what view ICU nurse has about factors that affect the time to extubation of cardiac surgery patients.A quantitative approach with descriptive and comparative design was used. Selection was all ICU nurses at a thoracic intensive care unit who were clinically active in patient care.The study showed that ICU nurses had good knowledge of why an early extubation was essential. However, the knowledge about the unit?s extubation criteria was low.

This essay aims to explore the popularity of health resorts through a patient?s perspective. What did the patients look for at the resort, which they could not find in the official health care? A basis for the analysis is Bonnie Blair O´Connor?s theory of Health Belief Systems.

AbstractBackgroundWorking in primary care with chronic leg ulcers is both time-consuming and difficult. There is a large category of patients with leg ulcers, that is expected to increase in a number of years, and many of them will probably come to a district nurse for help.AimThe aim of this study was to describe the district nurses' experiences of caring for patients with chronic leg ulcers in primary care.MethodThe approach was qualitative. The study was done with a phenomenological life-world approach. Seven district nurses working in primary care were interviewed. The phenomenological perspective focuses on the respondents? own life-world and has openness to the interviewee's own experiences.
"Visst gör det ont" Patienters upplevelse av postoperativ smärtbehanling
Introduction: A big part of the patients that recently has gone through operation at surgical units experiences pain in different scales after their operation. Untreated postoperative pain has been proved in different studies to increase to risk medical complications, longer rehabilitation, more days at the hospital and higher expenses for the hospital. One of the nurses? main tasks is to notice and relieve the pain of the patients. To engage the patients more in the medical process and make the nurses´ work more person-centered could make an impact on the postoperative pain and contribute to a better pain relief.

ABSTRACTAim: The aim of this study is to chart the experiences of Swedish general psychiatric care on a Swedish University hospital from a relative?s point of view. The study focus on how they are meet, satisfaction with and participation in the care. The study is done as an improvement project.Methods: The study was conducted by performing semi-structured interviews with eight close relatives to patients on a general psychiatric ward. The interviews lasted for 20-40 minutes.Results: The result shows that relatives are satisfied with the way they are meet by the staff on the ward, that they appreciate staff that are open, down to earth and are inviting to form a good relationship.

The purpose of this essay is to examine sfi (Swedish for immigrants), which is an ingrational-political tool with objective of teaching immigrants to read and write in Swedish. With the use of critical discourse analysis we examine the discursive practices within sfi. We also examine our methodological and theoretical approaches, and our application of them. Our research questions are as follows:? How are the discursive usage of ?person centered? and ?society centered? expressions being used?? How well does our methodological and theoretical resources work?In our theoretical viewpoint we use ?post colonial theory?, which is a perspective concerned with global power relations seen from a historical perspective.

Background:Studies show that when nurses are experiencing problems in the care of non-Swedish speaking patients, it is often due to obstacles in the communication. Studies also show that a prerequisite for adequate care is that there is a basis for a direct communication. Nurses perceive interpreters as an important link to the patient when they do not share a mutual language, the nurses perceive interpreters as a bridge in the conversation.Aim:The purpose of the study is to describe nurses' experiences of the quality of assessments done with the help of an interpreter.Method:The study was conducted through interviews with ten nurses, working at psychiatric clinics. Qualitative content analysis was used for the analysis, which resulted in five categories.Results:The analysis resulted in one theme: "it´s the interpreter who tells me and have not the feeling that the patient has" and five categories: "shades of the language", "conversation structure", "the interpreter's competence", "interpreting implementing" and "the interpreter's gender and origin". The nurses experience working with interpreters as a challenge.

The decision was taken by the regional assembly and resulted in that the citizens since thenhave had the opportunity to choose which health care provider they preferto stay with. Along with thisdecision a new market for private health careproviders was established, which resulted inincreasedcompetition between the providers.In order to manage the competition, the health care providers arenow forced to be more efficient in their operations andalso to beeven more focused on their patients.Backa Läkarhusgruppen is an example of a private healthcare provider in the region of Västra Götaland.In the same way it is also an example of anorganizationin need of analysing their operations to seehowit can be further optimized to meet patient needs and expectations.The aim of this study is to analyse Backa Läkarhusgruppen?s organization and its operationsin the hopeoffinding suggestions whichwill improve the patient flow.To allocate and solve the problems thatBacka Läkarhus suffer from,an approach inspired by Lean Healthcare is used throughout the report.Some of themost significant problems that were identified within the organization are:?The synchronization of demand and capacity is inadequate.?Absence of standardized routines.?Absence of present leadership.To be able to improve the patient flow Backa Läkarhus needs to handle these identified problem areas.By synchronizing the staffing level in relation to the trends in demand a reduction in the patient?swaiting times and throughput time will be possible. Creating clear responsibilities and routines for thestaff will contribute to a higher degree of patient focus and thus lead to a more efficient patient flow.

In the early 1990?s more and more companies of the Swedish public sector were exposed to competition, and this started the debate on how such actions might affect the business and even society.The first chapter examines how the public sector and especially health care, is financed and managed. We also explore what it means to be exposed to competition and find that there are many different ways of exposing the public sector to competition. The study examines dental care as an example of a market with both publicly and privately owned companies. The purpose of this paper is to explore how publicly and privately owned health care companies view their relationship with their consumers.
