Post Authorization Safety Studies (PASS) & Patient Support Programs (PSP) : Läkemedelsföretagens säkerhetsverktyg efter lansering

Introduktion: I juli 2012 uppdaterades regelverken för farmakovigilans inom Europa och flera förändringar gjordes i de bestämmelser som rör säkerhetsstudier på godkända läkemedel. En sammanställning av dessa behövdes för att lyfta fram viktiga bestämmelser och ge en tydligare överblick av de myndighetskrav som berör PASS (Post-Authorization Safety Studies) och PSP (Patient Support Programs). Syfte: Projektet syftar till att analysera och tolka regelverk och riktlinjer gällande PASS och PSP inom Europa samt utreda vad som gäller för PASS i de skandinaviska länderna. Detta för att lyfta fram viktiga bestämmelser och underlätta tolkningen för berörda företag. Material och metoder: Huvuddelen av projektet bestod av en litteraturstudie där fokus låg på lagar och riktlinjer inom Europa. Informationsinhämtning skedde även genom intervjuer med personal på Bayer AB och Läkemedelsverket. Jämförelser har sedan gjorts mellan svenska, norska och danska regelverk. Resultat: Myndighetskraven för PASS och PSP skiljer sig åt i vissa avseenden.

Hydropower is an important source of energy in Sweden and accounts for about 46 % of the total energy production. Sweden has about 1 200 hydropower plants and with the intention of raising the pressure head of the water and to store water, dams are built by the power plants. The storage reservoirs make it possible to adjust the energy production by demand and season. Dams in connection to the reservoirs are subjected to additional loading during periods of high floods or when an icecap in the winter is formed on the reservoirs. In order to maintain the dam safety, the power companies in Sweden are working with guidelines for dam safety from RIDAS.

The importance of good information security and safety grows larger and larger for each day in the information society of today. This concerns citizens, companies and public authorities. At the Swedish Transport Administration, the operation of an information security and safety process has evolved over the years to guarantee that the public authority can be trusted by its citizens, cooperating companies and other public authorities. This responsibility has been assigned to the Information Security and Safety Department (Informationssa?kerhetsavdelningen, ITsa?).In this thesis the aim is to review to what extent ITsa? achieves any business benefits for the Swedish Transport Administration, and to analyze, streamline and come up with solutions to improve their workflow.

Previous research and patient complaints to the Swedish Patient Support Committee shows that there is a lack of substance in the patient-nurse relationship. This may suggest that it can be difficult for health care to live up to health-care law which requires it to be of good quality, based on dignity and respect and designed so that the patient is an active participant and can make their own decisions when it comes to their health care. For a better understanding of what a dignified patient-nurse relationship is, this study aims to clarify what the patient values as good patient-nurse relationship and describe how they have experienced the patient-nurse relationship during their stay at an emergency care department. The study is a descriptive cross-sectional study with a quantitative approach, where the population consisted of patients who were cared for at an emergency care department in the County of Västmanland, Sweden during 2007. The sample consisted of 93 patients which were asked to answer a questionnaire.

Swedish healthcare are in need of radical changes to meet the requirements from both the government and the patients to create organisational working methods that better support the patients through their healthcare processes. Digitalization of patient data has been going on for decades in order to increase participation and understanding among patients in their care processes. However, availability of information does not ensure understanding. Therefore, resources should be devoted to create conditions to enable participation. The purpose of this study is to explore how visualization of patient data in the patient?s journal can affect the consultation in the encounter between the doctor and the patient.

Background: Postoperative infections are a serious Patient Safety problem and affects about 3 % of all patients undergoing surgery. The consequences are prolonged hospital stays, high social costs, increased use of antibiotics and impaired quality of life for the patient. It has been shown that postoperative infections may be caused by contamination already in the operating room. Today we know that there is growth of microorganisms on the instrument table´s top, but there is a research gap about the instrument table's edges.Aim: To examine if the sterile drape on the instrument table´s edges are sterile after finished surgery.Method: Contact plates are printed on strategically selected areas on the sterile drapes top and edges. In the pilot study, a total of 51 cultures were taken at the end of three operations which lasted over three hours.Results: Growth on the top surface and the edges of the sterile drape were detected.

Magnetic resonance imaging (MRI) is a safe and painless diagnostic method. The patient may become concerned, suffer from anxiety and experience the examination as unpleasant. Detailed information and a good treatment can reduce anxiety and distress for the patient. The purpose of this study was to illuminate the patient?s experiences in connection to MRI, and to describe the patients needs for information and treatment.

Introduction: To become acute ill and to be forced to seek treatment involves a difficult but also a vulnerable time for the individual. This acquires a good relationship between the nurse and the patient. At a surgical emergency ward patients with different diagnoses and trauma are treated. What distinguishes this specific type of ward is that care often occurs in a rapid rate and as a patient the environment may be experienced as stressed. The relationship with the nurse at the ward establishes a foundation for how the patient experiences the health care.

Background: The majority of the occupational accidents and work-related diseases at Swedish workplaces occur due to poor ergonomics during lifts and transportation of objects. The most common consequence of poor ergonomic conditions is low back pain (LBP) and the risk of developing LBP increases as the mechanical loading on the intervertebral disks grows. This study has investigated the biomechanical loading in the low back during two different types of patient lifts. The first lift (lift A) is developed by Modern Arbetsteknik (MA) and the second lift (lift B) is a common lifting technique among care workers. Objectives: The aim of the study was to identify any advantages or disadvantages with MA?s lift, regarding shear and compression forces in the L5/S1 disc.

This thesis is an evaluation of the new traffic solution at ?Skvallertorget?. The innovative theory behind it is to increase traffic-safety by introducing insecurity feelings for road-users. The conclusion of the evaluation is that the newtraffic solution is well adjusted according to guidelines for municipal traffic planning. The evaluation also illustrates the conflict between traffic planners deliberate attempt to avoid clear information and the road users wish to get it.

Effekter av sjuksköterskeledda interventioner i undervisning av äldre patienter med hjärtsvikt : en litteraturstudie

Patients with heart failure have a high mortality and often readmits to hospital care, due to a lack of compliance in the treatment plan. Patient education on self-care provided by nurses is important in improving patient and clinical outcomes.The aim of this study was to describe effects of nurse led interventions of patient education for elderly patients with heart failure. Literature study based on nine quantitative scientific articles. The results shows that different nurse led interventions of patient education with patients with heart failure had positive effects on self-care management, quality of life, health related quality of life and hospital care. E-mail reminder to community nurses and telephone education shows to be the most effective patient education interventions.

Personcentrerad vård beskrivs som att se hela människan och kunna sätta sig in i den andras livssituation. Respekt och förståelse för val och önskningar ska leda till självbestämmande och delaktighet i vård och behandling. Implementering av ett personcentrerat synsätt förordas för att möta de förväntningar som finns på god vård där patienten kan vara trygg och känna tilltro till vården. Att utvärdera personcentrerad vård är viktigt för att visa hur grundläggande behov som trygghet och tillit tillgodoses vid en förändrad vårdmodell. Syftet med studien var att belysa faktorer i personcentrerad vård som påverkar patientens upplevelse av trygghet och tillit.

In Sweden whenever trains or maintenance workers want access to the railway system, their requests of usage and movements are being supervised, granted and handled by train dispatchers. The profession holds a key role in making the complex railway system work in terms of efficiency, safety and delivering on-time arrivals. This master thesis, delegated by the Swedish Transport Administration, has been carried out at the Division of Human Computer Interaction at Uppsala University and at the train traffic control center in Gävle. The aim has been to map what information the train dispatchers use in order to make safety-related decisions concerning their working goals. And further to describe and analyze how the train dispatchers, visavi their tools and systems in the work domain, gather necessary information.The result indicates a complex, diverse and spread out setting of different programs, procedures and tools that make satisfactory fulfilment of efficiency, safety and punctuality in relation to the time table hard.

Gender aspects on spatial city planning In this thesis, theories regarding gender and spatial planning are applied to the comprehensive plan Fortuna och Hemgården in eastern Malmö. Independent of factors like class, ethnicity and age women experience lack of safety to a larger extent than men do. Today, safety aspects on housing projects is scarcely a controversial issue, quite the contrary. However seldom are the central terms such as safety and security defined. Research has shown that women feel safe when they experience little risk of being subjected to violence; security is deeply interconnected with feeling safe.
