Inledning: Forskning visar att smärta vid stora trauman är underbehandlat, vilket kan ge ett förlängt sjukdomsförlopp och leda till komplikationer för patienten. Smärtbedömning är en viktig del i smärtbehandlingen och kan göras med hjälp av observationer av fysisk status och smärtskattningsinstrument. Eftersom ett stort trauma även innebär en psykologisk påfrestning för patienten är det viktigt att lindra både smärtan och det psykiska lidandet.Syfte: Att beskriva ambulanssjuksköterskors uppfattningar om traumapatienters upplevelser av smärta samt beskriva ambulanssjuksköterskors erfarenheter av smärtbehandling och omvårdnadsåtgärder som lindrar lidande hos dessa patienter.Metod: En intervjustudie med elva ambulanssjuksköterskor. Intervjuerna analyserades med kvalitativ innehållsanalys.Resultat: Ambulanssjuksköterskans arbete vid smärtlindring vid stort trauma präglades av avvägningar och prioriteringar. Dessa gjordes utifrån ambulanssjuksköterskans egna bedömningar samt de riktlinjer som fanns inom ambulanssjukvården.

Syftet med denna litteraturstudie var att undersöka tillgänglig litteratur beträffande taktil beröring som alternativ smärtbehandlingsmetod samt vilka effekter taktil beröring har på patientens smärtupplevelse och för vårdgivande personal. Metoden som användes var Goodmans modell för evidensbaserad litteraturgranskning. Tio artiklar granskades enligt kriterier av Polit och Beck (2006) samt Willman et al (2006). Resultat visar vikten av beröring som metod för minskning av smärta, samt andra effekter som förbättrad sinnestämning och ökad livskvalitet hos patienter. Positiva och negativa effekter av beröringsbehandling för vård-personalen har tagits upp i några inkluderade studier..

Syftet med denna pilotstudie var att beskriva hur sjuksköterskor som arbetar som Case Managers uppfattar effekterna av denna arbetsmodell avseende medicinsk vård, social situation, missbruk, vård- och omsorgskvalitet samt samordning för patienter med missbruk och samtidig psykisk störning. Metoden var kvantitativ och genomfördes med en webbenkät. Resultatet visade att respondenterna anser att Case Management haft en positiv påverkan på patienternas medicinska vård och sociala situation. Det ledde också till en bättre kontakt med patienten, och förbättrad bedömning av patientens sammansatta situation. Sammantaget framkom en övervägande positiv uppfattning även av vård- och omsorgskvaliteten, samordningen och de arbetsmetoder som använts.

Efter att olika inlånade organisationsmodeller från industrin under 1930 till 1960-talet visat på brister när det gäller organisation av hälso- och sjukvård, introducerades i slutet av 1960-talet primary nursing. Modellen bygger på en till en principen, 24-timmars ansvar och minimal arbetsdelning, vilket leder till att sjuksköterskor ser hela människan och bedriver en patientcentrerad vård. Den rådande organisationsmodellen påverkar sjuksköterskans upplevelse och därför har vi utifrån vårt syfte, att beskriva sjuksköterskans upplevelse av primary nursing, valt att göra en litteraturstudie. Med en kvalitativ innehållsanalys med manifest ansats har vi analyserat data från tio artiklar. Analysen resulterade i fyra kategorier: att känna ansvar och vara självständig, att samarbete och kommunikation är avgörande, att patienter och anhöriga är nöjda och trygga, att känna tillfredställelse i arbetet samt krav och hinder.

Purpose/aim: My purpose is to illustrate the significance of the language in communication with immigrants having a different native language, who suffers from dementia. The three main questions have been:How important is the native language in the communication with immigrant who suffers from dementia?What consequences can happen for the immigrant who does not understand Swedish? How can we use the salutogenesis point of view to create satisfactory care for immigrants who has difficulties to make them self understood in the Swedish language? Material/method: I made qualitative interviews with staff working in nursing homes with dementia care. I also searched scientific articles and other findings in this area of science. Main results: The best care for immigrants with dementia is nursing homes where the staff speaks the immigrant?s native language.
