Anestesiologisk omvårdnad av patient med obesitas

ABSTRACTBackground: Obesity and overweight are health issues that more than every tenth person around the world suffers from and the number of individuals with BMI >30 is every growing. As a result, more patients that are subjects to anesthesia will have an increased BMI. Changes in respiration, airway anatomy, altered drug metabolism and multiple physiological changes in the body makes general anesthesia, of the patient with obesity, an anesthesiological challenge.Purpose: The aim of this study is to identify the specific anesthesiological nursing care of the patient with a BMI >30, undergoing general anesthesia.Method: A research plan is created and to test its credibility, a pilotstudy of 15 surveys in two surgical departments will be conducted. This study has a qualitative inductive design and the data collected through semistructured questionnaires. The result was analyzed using qualitative content analysis.Result: The result showed a clear focus on three main areas; airway management, pharmaceuticals and positioning of the patient.

abstractBackground: Dementia affected patients have increasing problems with memory , impaired language ability, which affects communication and behaviour is considerable. These difficulties may provide an outlet for aggression, agitation and anxiety, and are common in dementia, which is exhausting for patients and their relatives and carers. Behavioural changes are common condition in patients with dementia and affects quality of life and cause unnecessary suffering . Pharmacological treatment that is inserted to relieve these symptoms have many side effects. Printed out more knowledge about non- pharmacological nursing interventions that can facilitate care for patients , relatives and carers are great.Aim: To describe non- pharmacological nursing interventions in dementia care .Method: A general literature review of thirteen scientific papers compiled and described in a result.Results: Compilation resulted in two themes,  Music in dementia care and Stimulation in dementia care and four subthemes, Singing in the care, Listening to music, Activities and Consolation and fillip that showed a dominant positive effect on dementia affected patients and carers and there by create opportunities for caring in dementia care .Conclusion: The different nursing interventions may be useful in dementia care when they show a dominant positive effect on various behavioural changes, it seems memory stimulant and promotes communication between patients and carers which favours caring .Keywords: Agitated behaviour, dementia, non- pharmacological, literature review, nursing. .

Experiences and needs of family members in the end of life care at a nursinghome

Background: There might be a risk that family members experience limited possibilities to act, participate and influence the end of life care for the elderly in the nursing home. For family members it could be the first time they face death which can imply a lot of questions and existential needs. Aim: The aim of this study was to examine the needs of family members in the patient terminal care in a nursing home. Research methods: The study implemented a qualitative approach and data was collected through four interviews and through a literature review were 17 articles were selected. The results were processed and analysed with a qualitative content analysis and two main categories, participation and assurance with subcategories were identified.

The aim of the literature review was to investigate how relatives to a person suffering from dementia, in institution, experienced the nursing hom staff and the relation to them. The study was based on eight scientific articles. The result gave thirteen themes, that showed that relatives experienced the nursing home staff having a hard work under pressure. They were nice but without knowledge. The relatives experienced that the nursing home staff didn´t see or hear them and gave no confidence.

The nurse may use various assessment tools to highlight and document patients at risk for developing pressure ulcers. The purpose with this study was to examine which opinions and knowledge the directors have about the physical residential environment's importance on accommodations for people with dementia disease and how they do to create and to maintain a good physical residential environment. The issues have been: Which opinions and knowledge the directors had about the physical residential environment's importance for the people with dementia? How did the managers work in order to create and maintain a good physical residential environment? We had four themes, homelike environment on the accommodations, priorities on the accommodation, care/nursing in the physical residential environment and the manager?s possibility to influence the physical residential environment. The study followed a qualitative method. Five managers were interviewed on four accommodations and the managers were said to work actively with the physical residential environment.

Background: Examinations with the intent to assess neurological functions and level of conciousness are common in neurocritical care, but the effect on the patient is not entirely researched.Aim: The purpose of this study was to describe the patient?s situation at the neurological wake-up test concerning the environment, nursing interventions and the possibility given the patient to become awake, and if these conditions affect the outcome of the wake-up test.Methods: A quantitative observational study took place at a neurocritical care unit, where 20 patients were observed from interruption of sedation to the neurological assessment, which was performed by a neurosurgeon or a specialized critical care registered nurse. Nursing interventions, environmental factors and physiological parameters were documented.Result: Twelve intervention varieties to control patients? physiological parameters were used. CPP > 80 mmHg was found in 90 % of observations.

The aim of this study was to describe home care personnel and their experiences in palliative care. The design was qualitative and data were collected through interviews. Five home care personnel were interviewed, all women, whom have experience of palliative home care.The material was then analysed with qualitative content analysis and eight main categories, derived from experiences of home care personnel, were identified: relationship, safety, quality of care, a better end-of-life, routine, information, knowledge and competence, and work environment. The result showed that all participants of this study identified themselves as family members of the patients whom they had cared for during a long period of time. Additionally, the participants were emotionally touched when the patient died.The majority experienced that the patients do receive good care in their homes.

There are fixed norms and rules which the elderly more or less have to adjust to. The aim of this study was to describe how nursing staff attitudes influence on elderly care takers meal situation. The method of this study was a literature review. The result showed that norms and rules not always are adjusted to caretakers? wishes and needs.

Nowadays there?s a high pace in health care where many decisions have to be made during time pressure, a lot of information has to be transferred between health care units and sometimes complicated equipment are used. Cases of mistakes are reported more than ever to the national committee responsible for health care. Many of the mistakes, where the nurses are involved are due to the nurses? stress.The aim of this study was to gather information to describe causes of stress in nursing care and the nurses? experience of stress influence on health care.The method that was chosen for this study was a literature review where scientific, critically reviewed articles were analyzed.  The analysis resulted in two main themes.
