Background: Hand hygiene is the simplest and most fundamental means to prevent care related infections. It is of great importance that the guideline of hand hygiene is followed for the patient?s sake. Unfortunately, healthcare workers perform hand hygiene too seldom. If the patient comes down with a care related infection, the suffering will increase.

Att beskriva och jämföra en expertgrupp och intensivvårdssjuksköterskors överensstämmelse i att detektera delirium hos intuberade, respiratorbehandlade patienter med sedering/analgesi, före och efter en utbildningsintervention : En kvasiexperimentell stud

The aim: was to describe and compare a group of experts and critical care nurses' agreement in detecting delirium in intubated, ventilator treated patients with sedation / analgesia, before and after an in house training intervention with the instrument Confusion Assessment Method for the Intensive Care Unit (CAM-ICU).Method: A quasi-experimental study, one group pretest - posttest design. A convenience sample of 17 critical care nurses in a general intensive care unit included. To detect delirium the instrument CAM-ICU was used, 21 paired tests before and 22 after an educational intervention.Main Results: The results showed that after an in house training intervention sensitivity and kappa coefficient improved of the characteristic 1 "acute onset and fluctuating course," an improvement that was significant. In other features, and overall values were signs of numerical improvement and deterioration in sensitivity, specificity and kappa coefficient but no significant change.Conclusion: Implementing a new instrument for detecting delirium in clinical practice requires education and follow-up. A small sample of critical care nurses with varying ability to use the new instrument and the fact that patients' status may change rapidly making it difficult to draw any conclusions from this study.

Enteral nutrition vid palliativ vård. En systematisk litteraturstudie

Syftet med denna litteraturstudie är att undersöka vad omvårdnad innebär utifrån ett sjuksköterskeperspektiv och etiska faktorer gällande enteral näringstillförsel vid palliativ vård. Forskningsansatsen har varit en systematisk litteraturstudie baserad på Goodmans sju steg i forskningsprocessen. Studiens resultat baseras på 10 vetenskapliga artiklar. Utifrån dessa artiklars resultat utkristalliserades olika teman såsom riktlinjer, dokumentation, åtgärder, komplikationer och effekter inom området omvårdnad kring enteral näringstillförsel. Resultatet påvisar att enteral nutrition ingår i sjuksköterskans ansvarsområde.

Strategier för en gräsrotsbyråkrat : en intervjustudie av biståndshandläggning i äldreomsorgen

In our paper we chose to study care management in the old age care. According to the law for the social services the needs of elderly should govern the assistance they are granted. But research has shown that municipal guidelines govern the assistance and that the elderly have to adjust their needs to them. Thus there is a discrepancy between law and practice. The aim of this study was to understand how care managers relate to municipal guidelines, laws and the requests of elderly when the application for assistance is established.

In today?s health care it?s most common to treat an illness with a kind of pharmaceutical preparation. But lack of resources, given information and bad communication and collaboration between the patient and the nursing staff can lead to a patient unwilling to follow the doctor?s prescription. This does not just affect the individual patient?s health but it can also result in consequences for other people and the whole society.

Nutrition och bensår

Patienter med bensår är vanligt förekommande i vården och nutrition bör vara en del av behandlingen. Sjuksköterskans kunskap och attityd till nutrition kan således påverka omvårdnaden av bensårspatienter i hög grad. Syftet med denna litteraturstudie är att belysa nutritionsstatus och läkning hos bensårspatienter samt sjuksköterskans kunskaper och attityder om nutrition. Metoden är en studie där tio artiklar har granskats utifrån vetenskaplighet och kvalitet. Resultatet visar att många bensårspatienter ligger i riskzonen för undernäring och har brist på ett eller flera näringsämnen.

 The number of hidden refugees in Sweden is estimated to be at least 15,000. The law, which only allows this group a very limited access to health care, can be considered to clash with the human rights and the ethical codes related to the health care professionals.The aim of the study was to examine how the personnel in public health care may experience treating hidden refugees and which ethical conflicts that may be connected to this. The study, which is of a qualitative descriptive design, is based on eight semi-structured interviews. The interviewees were trained nurses and mid-wives in an emergency room, a maternity ward and a health care centre for asylum seekers.The experience of treating hidden refugees amongst the informants was limited. The study proved that the knowledge of laws and guidelines regarding hidden refugees amongst the interviewed health care personnel was poor.

Aim: The aim of this study was to describe intensive care nurses? experiences communicating with patients during mechanical ventilation.Methods/design: A qualitative interview study. Interviews where analyzed using descriptive content analysisSetting: Nine intensive care nurses from two different intensive care units were interviewed using a semi structured interview guide.Background: Past research has shown that patients during mechanical ventilation in the intensive care unit, feel very vulnerable and the helplessness of being unable to speak. These patients feels that they are completely dependent on the nurses and their competence. It has been shown to be very important that the patient feels included, acknowledged and respected.Results: The analyzed data resulted in a theme; through communication strive to preserve patients´ dignity and three main categories; create relationship to the patient, minimize patients´ vulnerability and don´t give up.

Compliance hos njurtransplanterade

Bakgrund: Att följa restriktioner gällande livsstil, och att ta mediciner som ordinerat, är nödvändigt för att en njurtransplanterad patient ska bibehålla funktion i den transplanterade njuren. Inom sjukvården benämns detta beteende för compliance. Non-compliance hos njurtransplanterade rapporteras i studier uppgå till 43 %. Non-compliance rapporteras öka risken för avstötning av njuren med upp till sju gånger för en patient med låg compliance, jämfört med en patient med hög compliance. Syfte: Syftet med aktuell litteraturstudie är att belysa faktorer som påverkar en vuxen njurtransplanterad patients compliance.

Early extubation of cardiac surgery patients has become increasingly important. The assessment of the patient before an early extubation is crucial and the intensive care nurses (ICU nurses) in this estimation is there for very important.The aim of this study was to examine critical care nurses' knowledge of early extubation, and what view ICU nurse has about factors that affect the time to extubation of cardiac surgery patients.A quantitative approach with descriptive and comparative design was used. Selection was all ICU nurses at a thoracic intensive care unit who were clinically active in patient care.The study showed that ICU nurses had good knowledge of why an early extubation was essential. However, the knowledge about the unit?s extubation criteria was low.

Faktorer som påverkar sjuksköterskans omvårdnad av mrsa bärande patienter

Mulitresistent staphylococcus aureus (MRSA) is a bacteria that is resistent against penicillin, this makes it difficulte to treat. MRSA causes woundinfections and other seriouse conditions. If MRSA establishes in an hospital environment it often spreads fast. Common ways of spreading MRSA is through direct or indirect contact. The nurse should work according to guidelines, apply hygienic routines and prevent the spread of infections.
