Att vara i en intensivvårdsmiljö - patientens perspektiv

The Intensive Care Unit (ICU) is a special unit for patients who is in the need for emergency life-sustaining measures and every patient has complex medical- technical equipment. The experiences of a critical illness and the needs of critical care are experienced in different ways. The purpose of this study was to describe the experiences from patients in the ICU and how this experience has an influence on the patients. The method was a study of literature with a qualitative inception based on eight scientific articles and the analysis of content has been inspired by Graneheim and Lundman (2003). The result of the study is presented by four categories: to loose the ability to make themselves understood, to be forced to adjust to the environment of caring, to have the lack of connection with reality and to be in an intensive care environment, The results showed that patients in the ICU are affected by the attitude from the staff, the equipment around them as well as the sounds and lights in the environment there.

Suicide and attempted suicide frequently occur in society today and represent a major public health problem. In inpatient psychiatric care the nurse have a central role when caring for suicidal patients. Few systematic studies exists that address the nurse?s practical work with these patients. The purpose of this study was therefore to describe the nursing care of suicidal patients as it was perceived by registered clinical expert nurses.

Can coercive care be justified against drug and alcohol addicts or should treatment be offered on a strictly voluntary basis? This was one of the central questions debated in Sweden during the 70's when the development of a new social legislation, The Social Services Act (Socialtjänstlagen), took place. The first aim of this thesis was to analyse whether the arguments that were presented both in favour for and against coercion were sound. To achieve this, a method for analysing arguments was employed, where a sound argument is defined as acceptable, relevant and adequate. The analysed material consists of both official government reports and books of influential debaters.

ABSTRACTBackground and aims: Nutrition is a part of the medical treatment, and also a part of the nursingcare. In spite of this, malnutrition is a common problem in the health care sector. Studies show thatnurses have too little knowledge about nutrition and malnutrition and that nutrition has low priorityin the daily work. The aim with this study was to find out the nurses self-reported knowledge aboutand their attitudes towards malnutrition, nutrition screening, prevention and treatment of patientswith malnutrition.Methods: A questionnaire was handed out to 65 nurses in two different care units and 36questionnaires were answered.Results: The respondents from the two care units self-reported their knowledge about nutrition andmalnutrition low. Most of the respondents stated to have a big need of education about nutritionscreening and more than half of the respondents stated that they have a need of education forprevention and treatment of malnutrition.

Introduction: Palliative care is based on an active total care where the patient's independence and participation are included. Few studies have been based on a concept of freedom in relation to this target group.Aim: The aim of this study was to describe the perception the nurse has about "freedom" - its significance and meaning from an existential perspective for patients with palliative care needs.Method: A qualitative content analysis based on a narrated and written down text from focus groups with adequate examples. The starting point in the focus groups was to focus on existential issues in relation to patients near death. Based on the concept of freedom, has the printouts been analyzed, categorized and codedFindings: Nature, identity, integrity and self-esteem are important aspects for our approach to freedom but freedom is also about existential questions where hope, reconciliation, freedom and forgiveness are included.Discussion: Discussion: We all have our own definitions and meanings of freedom. Abstract This essay is about the visibility of everyday life in a nursing home. The thesis is based on the following questions: What characterizes the social life and conversation between staff and residents? What content has the social life and the conversation between these parties? What is the power relationship between the staff and residents? To answer these questions, we used the qualitative approach with open observations, along with interviews. The study is partly based on direct observation by an observer as a participant in a special housing, and on semi-structured interviews with seven respondents, three of them staff and four residents in the same nursing home. For the results and analysis section, we use the Grounded Theory as a method which constructs a parallel between data processing and analysis of the interviews, where we first encode, categorize and create themes.

Through semi-structured interviews with 8 school photographers and 10 parents,this study attempted to clarify what photographers and parents of children(aged 1?5) in preschool think of photography business in the current situationprovides retouching children?s school photographs and what they considers tobe acceptable. The result shows that companies do not offer photo retouchingfrequent, but performs this service at the direct request of the customer. Themajority of parents agreed that dirt and food was accepted to be removed fromthe photograph, and that this does not affect the child's self image. Background: Women are recommended to breastfeed exclusively for six month; despite this the frequency of breastfeeding duration is decreasing in Sweden. Research indicates that there is an association between attitudes towards breastfeeding and perceived difficulties and initiation and duration of breastfeeding. The aim: to study women?s attitudes to breastfeeding and their perceived difficulties with breastfeeding expressed on parent oriented websites on the Internet. Att beskriva och jämföra en expertgrupp och intensivvårdssjuksköterskors överensstämmelse i att detektera delirium hos intuberade, respiratorbehandlade patienter med sedering/analgesi, före och efter en utbildningsintervention : En kvasiexperimentell stud

The aim: was to describe and compare a group of experts and critical care nurses' agreement in detecting delirium in intubated, ventilator treated patients with sedation / analgesia, before and after an in house training intervention with the instrument Confusion Assessment Method for the Intensive Care Unit (CAM-ICU).Method: A quasi-experimental study, one group pretest - posttest design. A convenience sample of 17 critical care nurses in a general intensive care unit included. To detect delirium the instrument CAM-ICU was used, 21 paired tests before and 22 after an educational intervention.Main Results: The results showed that after an in house training intervention sensitivity and kappa coefficient improved of the characteristic 1 "acute onset and fluctuating course," an improvement that was significant. In other features, and overall values were signs of numerical improvement and deterioration in sensitivity, specificity and kappa coefficient but no significant change.Conclusion: Implementing a new instrument for detecting delirium in clinical practice requires education and follow-up. A small sample of critical care nurses with varying ability to use the new instrument and the fact that patients' status may change rapidly making it difficult to draw any conclusions from this study.

Teaching in pre-school usually takes its starting-point in children?s reality and curiosity. Apart from that it has also been important to take contains of a special subject under consideration, for example development of language and communication or mathematics development. The purpose of this study was to investigate how natural science as a special subject in pre-school education is expressed by five trainee teachers when they teach children at the age of 2-7. Generally the study focuses on the students? teaching actions as well as their line of argument about their teaching and the choices they have to make.

Införande av evidensbaserade metoder: ett kommunperspektiv

This thesis is composed within the framework of the supplementary course in psychotherapy concentrating on family therapy (60 credits) arranged by Lund University's affiliate in Västerås, Sweden.The purpose of this thesis has been to explore how the idea to initiate evidence based methods in the workings of the Individual and Family Care Department of the City of Västerås was born. Furthermore the thesis will explore if consideration for methods of theoretical implementation are implied. The study assumes a qualitative attempt and is based on semi-structured interviews from three levels within the organization, including both management and operation.Theories on organization,implementation and the theory on change strategies have been used as a theoretical starting point.The study result shows that introduction of evidence based methods in the City of Västerås is based on the experiences of the organization regarding the need to develop and increase social services contributions to non-institutional care. Experiences from expensive transferrals to institutions which only occasionally produced satisfactory results were a contributing factor. New published research and a strong interest and desire inside the department to improve and change social services contributions have naturally been a positive influence for the development.

Valfrihetens konsekvenser : medvetna och omedvetna skolval

This study illuminates chosen parts of the present opportunities, in the Western World, for parents and children to choose which school the child will attend (freedom of choice), along with possible consequences that may follow. My research questions are: 1) Do tendencies show that freedom of choice leads to increased segregation, and if that is the case, how is it manifested and does it matter? 2) What relations between the student composition of schools and teachers' expectations and attitudes toward students are there? 3) Do the results from previous studies match with the result from my interviews? The methods used for answering the questions are literature studies and interviews made with six teachers who are working in two upper secondary schools in the northern part of Sweden, i.e. three from each school.The result shows that freedom of choice, regarding schools, seems to lead to student compositions that contribute to increased segregation in societies. It leads to more homogenous student compositions, in the form of ethnicity and social class, in schools in general, with consequences like different expectations and attitudes among teachers depending on the status of the composition.
