Efter att olika inlånade organisationsmodeller från industrin under 1930 till 1960-talet visat på brister när det gäller organisation av hälso- och sjukvård, introducerades i slutet av 1960-talet primary nursing. Modellen bygger på en till en principen, 24-timmars ansvar och minimal arbetsdelning, vilket leder till att sjuksköterskor ser hela människan och bedriver en patientcentrerad vård. Den rådande organisationsmodellen påverkar sjuksköterskans upplevelse och därför har vi utifrån vårt syfte, att beskriva sjuksköterskans upplevelse av primary nursing, valt att göra en litteraturstudie. Med en kvalitativ innehållsanalys med manifest ansats har vi analyserat data från tio artiklar. Analysen resulterade i fyra kategorier: att känna ansvar och vara självständig, att samarbete och kommunikation är avgörande, att patienter och anhöriga är nöjda och trygga, att känna tillfredställelse i arbetet samt krav och hinder.

Purpose/aim: My purpose is to illustrate the significance of the language in communication with immigrants having a different native language, who suffers from dementia. The three main questions have been:How important is the native language in the communication with immigrant who suffers from dementia?What consequences can happen for the immigrant who does not understand Swedish? How can we use the salutogenesis point of view to create satisfactory care for immigrants who has difficulties to make them self understood in the Swedish language? Material/method: I made qualitative interviews with staff working in nursing homes with dementia care. I also searched scientific articles and other findings in this area of science. Main results: The best care for immigrants with dementia is nursing homes where the staff speaks the immigrant?s native language.

SAMMANFATTNING Detta examensarbete syftar till att kvalitetssäkra omvårdnaden av hästar med medicinsk kolik på djursjukhusens stationärvårdsavdelning. Genom att bidra med ett förslag till standardvårdplan för hästar med medicinsk kolik vill vi optimera hästens vård och omvårdnad under sjukhusvistelsen. Syftet med att arbeta med standardvårdplaner (SVP) är att skapa högkvalitativ omvårdnad, underlätta dokumentationsarbetet och i förlängningen kunna få mer tid till att se och vårda den enskilde patienten. Kolik är en åkomma med många olika samverkande faktorer, både miljöbetingade och eventuellt ärftliga Kolik i matsmältningskanalen delas ofta in i medicinsk- och kirurgisk kolik och i detta arbete läggs vikten på medicinsk kolik. Huvuddelen (~90 %) av all kolik kräver ingen kirurgi och tillhör därmed gruppen medicinsk kolik. Kolikavsnittet i arbetet är tänkt som en kunskapsöversikt över sjukdomstillståndet och ska användas som ett styrdokument för att trygga en kvalitativ basnivå på omvårdnaden. I standardvårdplanen har ett antal riskområden identifierats och därefter har riskdiagnoser, mål och åtgärder för att uppnå huvudmålet - en kolikfri patient, utarbetats. Därpå utvecklades kryssprotokoll för parametrar vid observation av kolikhäst och för smärtbedömning vid medicinsk kolik. Nearly nine percent of the population in Sweden are 75 years or older. Still they consume more than a quarter of all medicines. This increased use, is specially noticeable in nursing home residents, where the elderly is using a average of 8-10 preparations per day. According to the National Board of Health and Welfare in Sweden, the number of drugs and specially psychotropic drugs, has proved to be a risk factor for drug-related problems among the elderly, such as drug interaction and side effects. Routines for regular and systematic follow-up of drug prescription and utilisation reviews were not extensively implemented.
