Anestesipersonalens peri-operativa omvårdnad av överviktiga och obesa barn

Children are a group of patients that require special knowledge from the anesthesia staff, both general and specific. Theoretical knowledge and practical experience is necessary to adequately care for the child as it anatomically, physiologically and mentally may differ from adult patients.A growing problem in the pediatric population is overweight and obesity. From a stagnation in the early 2000s the prevalence of overweight and obesity among children in the world are again increasing.The purpose of this study was to examine peri-operative care of overweight and obese children.Systematic searches were done in PubMed, CINAHL and Scopus. After the quality audit according to SBU, twelve articles were included for analysis.Results suggest that overweight and obese children have a higher risk of respiratory peri-operative complications. The risk of hospital admission after outpatient surgery was also greater in this group as well as higher costs for health care.Overweight and obese children are complex subjects with more frequent peri-operative risk factors than normal-weight children.

Background. Anesthetic-related death in small animal anesthesia is about ten times more common than in human anesthesia. Recent research identifies the early postoperative period as particularly risky and highlights the need for regular and careful monitoring of the veterinary patient during this period. Objective. The aims of this study were to investigate whether there are standard procedures in terms of regulated routine patient observations and record-keeping during the early postoperative period at Swedish veterinary clinics, and if there are any guidelines for monitoring the veterinary patient during recovery. Methods. A small survey including ten Swedish veterinary clinics was carried out as well as a literature review. Results.

Kartläggning av preoperativa fasterutiner på tre vårdavdelningar

The aim of this study was to investigate for how long patients fast before thoracic surgery, plastic surgery and ear, nose and throat surgery, at the University hospital of  Uppsala, and what knowledge patients have about fasting and how they get affected by it. 30 patients were interviewed, 10 patients at each ward, according to a questionnaire. Data processing was performed with SPSS. There was no difference in fasting time for solids between the wards but the mean fasting time was 14 hours. In the thoracic ward patients fasted from liquids considerably longer than in the other wards.

Anestesi och intensivvårdssjuksköterskans hantering av mekanisk ventilation : - en litteraturstudie om minskning av atelektasbildning och förebyggande av komplikationer

Background Atelectasis is a commun respiratory complication related to general anesthesia and may contribute to pneumonia and acute respiratory failure. Atelectasis apear within 10 minutes after anesthetization and occur in 90% or more anesthezied patients regardless of age and gender. The aim of this study was to indentify if there are any evidence for which methods are ultimate or are most efficacious to reduce and prevent the formation of atelectasis.                                                                                                          Methods This study is a descriptive litterature study where 28 scientific studies have been indentified for review and analysis.                                                                                                               ResultsThe use of 100% oxygen during the induction of anesthesia favors the developement of atelectasis. This can be prevented by preoxygenation with oxygen content of 100% combined with a PEEP of 6-10 cmH2O. When free airway is secured, oxygenation with 100% was found to be unnecessary.

ABSTRACT Introduction: One of the great difficulties that the nurse anesthetist encounter in their profession is to assess and estimate sleep depth versus signs of pain in the anesthetized patient. Based on a pre-operative appointment and the perioperative monitoring, an assessment of the patient is made. The experience that the nurse anesthetist has can make it easier to understand what subtile signs of the patient depends on. Purpose: With this in mind, the purpose of the pilot study was to try to find out if there are specific signs of pain during anesthesia and if the professional experience played a role. As a subsidiary aim the authors examined whether there were factors in the patient that the nurse anesthetist takes into account in the identification of pain during anesthesia.

Alternativ till obedövad kastrering av smågrisar : - Ur ett ekonomiskt och djurhälsomässigt perspektiv

This degree project aims to evaluate alternative methods to the unanaesthetized castration of piglets. Castration of piglets is performed to prevent the development of boar taint. Boar taint can cause great financial losses for the pig manufacturer. Unanaesthetized castration of piglets is not regarded to be satisfactory from an animal welfare perspective and the procedure will for that reason be outlawed in the near future.Several alternative methods are used in countries all over the world with promising result. The purpose of this study is to evaluate if this methods could be applied in Swedish pig production.A literature study has been combined with interviews; field studies, questionnaires and a small study on male pig production.To be applicable to and applied in Swedish pig production the alternative method must be financially accepted by the manufacturers but also accepted from an animal welfare perspective.Castration is the only way to guarantee a meat free of boar taint.

Lameness localized to the hoof is a common problem in horses. Magnetic resonance imaging (MRI) as a diagnostic tool is becoming more common and is well suited for examination of the distal limb and the hoof capsule. Today there are two different methods of MRI where in one the horse is under general anesthesia and in the other the horse is standing and only sedated. In this study we have compared the visualization of the distal interphalangeal joint using these two methods in two different groups of horses with unknown clinical problems referred to two different equine hospitals. The width of the joint structures including thickness of joint cartilage and joint fluid and also the width of the dorsal pouch of the distal interphalangeal joint were measured.

Ambulatory surgery is common and develops alongside surgery and anesthetic methods. Time admitted is short and aftercare takes place in the home environment.Objective: The aim of the study was to investigate which symptoms patients experience after ambulatory surgery related to anesthesia. Furthermore how strongly symptoms affect the patient. Symptoms appear as a result of anesthesia and surgery and can therefore be interpreted as nursing induced suffering.Method: Participants were recruited at ambulatory clinics at a Swedish university hospital spring, 2015.  Prior to the study permission was granted from the clinic managers. The study is a quantitative descriptive longitudinal study.  Data collection forms were administered and data collection was performed by phone.

Användarorienterad design av anestesisystem

In inhalation anesthesia life-support systems are used to provide gases to the patient and to monitor important parameters. These systems are used in dynamic and stressful situations where the information-load on the operators often is significant. This thesis presents a design concept of an anesthetic system. The design goal has been to keep the information-load on the operator low with the objective to minimize confusion and error.A number of contextual inquiries were made in the operating room during surgery at four hospitals in Sweden. The goal was to understand the actual use-situations when anesthetic systems are employed.

När återhämtar patienten sig snabbast? : Jämförlse mellan inhalationsanestesi och total intravenös anestesi.

Generell anestesi kan ges som inhalationsanestesi eller total intravenös anestesi (TIVA). En förutsägbar anestesi med snabbt uppvaknande och bibehållen vakenhet är en högt önskvärd egenskap oavsett anestesiform. Det råder en klinisk och vetenskaplig diskussion om vilken anestesiform som ger snabbast tidig postoperativ återhämtning. syftet med studien var att jämföra patienters tidiga postoperativa återhämtning efter inhalationsanestesi respektive efter total intravenös anestesi (TIVA). Metoden var en litteraturstudie baserad på 15 vetenskapliga artiklar.

Sjuksköterskans postoperativa omhändertagande av patienter som genomgått generell narkos.

Tidigare forskning visar att patienter har olika behov postoperativt, det är sjuksköterskans uppgift att anpassa omvårdnaden till individen. Patienter har postoperativt ett extra stort behov av information. Sjuksköterskor och patienter har olika åsikter om vilka behov som ska tillgodoses först. Sjuksköterskan anser att kompetenta vårdgivare, smärtlindring och den behandling som vårdpersonal ger ska prioriteras först. Det viktigaste för patienterna var kompetenta vårdgivare, kommunikation samt god omvårdnad.

Dagkirurgiska patienters upplevelser av anestesi och kirurgi

Dagkirurgi blir allt vanligare. Då det genomförs under generell anestesi förekommer ofta oro och postoperativa komplikationer som smärta och illamående. Patienters tillfredsställelse med dagkirurgi och anestesi har tidigare studerats, men ofta med enkäter av låg kvalité. Syftet med denna studie var därför att med hjälp av intervjuer studera dagkirurgiska patienters upplevelse av anestesi och kirurgi. Sju patienter intervjuades.

Perioperativ omvårdnad vid kejsarsnitt hos tik ? förbättra utfall för tik och valpar

Dystocia, or difficult birth, is a commonly seen complication to natural birth in veterinary hospitals. Although other methods of resolving dystocia exist, 60 % of dystocias will need to undergo caesarean section. To perform a caesarean section, the bitch will have to undergo anesthesia. The purpose of this literary study was to provide a deeper understanding regarding how the physiologic changes that occur during pregnancy influence anesthetic management in the bitch and puppies. Which anesthetic drugs and drug combinations indicated was also investigated as well as how a veterinary nurse can optimize care of the mother throughout the procedure and how resuscitation of the puppies should be carried out for best possible outcome.

Anestesisjuksköterskans stöd till den födande kvinnan : Vid planerat kejsarsnitt i spinalanestesi

Bakgrund: Kejsarsnitt har blivit vanligare, orsaken kan vara medicinska indikationer, men också förlossningsrädsla. Att kvinnor som föder barn vaginalt får en bättre upplevelse om de får stöd av en medmänniska är välkänt. Studier av kvinnor som föder barn med planerade kejsarsnitt visar att de också behöver stöd. Detta stöd ges framför allt av anestesisjuksköterskor eftersom barnmorskan är sterilklädd.Syfte: Syftet var att beskriva hur anestesisjuksköterskan ger stöd till den födande kvinnan vid planerat kejsarsnitt i spinalanestesi.Metod: En empirisk, deskriptiv studie med kvalitativ ansats genomfördes. Fem anestesisjuksköterskor intervjuades.
