Kalkyler i vården: en översikt och ett praktikfall

This thesis investigates management accounting systems in a Swedish health care environment. Through a qualitative case study of an activity-based cost accounting system that was introduced in 1994 at S:t Görans Sjukhus, a large Stockholm hospital, the following questions are addressed: What benefits can an activity-based cost accounting system offer the Swedish health care providers? What are the potential problems impeding the implementation of such a system? Main findings: an activity-based cost accounting system can improve the understanding of where costs are generated in the production processes. Comparisons between alternative production processes are improved, and the data may serve as a basis for internal pricing measures. Concerning the second question, difficulty in reconciliating the principles of economic control with the culture present in a hospital is identified as the main obstacle to a new management accounting system..

Beskrivning av patienters postoperativa vårdförlopp tre dagar efter kolorektalkirurgi enligt ERAS vårdprogram

AbstractThe purpose of this study was to describe the post-operative care during the three first days for patients who have undergone colorectal surgery according to ERAS care programs with a focus on the variables nutrition, elimination, activity, type of analgesia and the number of hospital days documented in the patient record and patient log books. ERAS means "Early Recovery After Surgery" and the purpose of the health care program is to accelerate recovery after colorectal surgery. The study had a descriptive design and a quantitative approach, in which 51 patient records were included. Log books and patient records were reviewed postoperatively. According to the log books estimated most of the patients, who had documented, that they ate and drank very good or good.

There are several different kinds of bacteria that has developed resistance against the antibiotic agents used in todays medicine. Most of them has evolved from the human health care but several of them has gone over to also cause problems in the animal health care. They often occur as nosocomial infections and the greatest risk of exposure to them is during health care. Since infections with a resistant bacterium can be difficult to treat, it is desirable to prevent this spread, especially since an animal treated in a hospital often is more susceptible to infection, due to their primary condition. In Sweden the current resistance situation is good and is yearly monitored by the Swedish Veterinary Antimicrobial Resistance Monitoring (SVARM) programme.

Several professionals with various skills expected working together in the traumateam at the care of the traumapatient. At the same time, research shows that substandard teamwork leads to avoidable errors in care. One way to improve teamwork is to practice. Purpose: The purpose of this study is to shed light on the experiences of the traumateam exercises that exercise participants feel has been favorable for the patient at the initial care of trauma patients.

Background: Delirium is a common condition among elderly patients who are being treated in a hospital environment. However, a large number of patients are wrongly diagnosed. The highest risk categories to develop delirium are elderly people and postoperative patients. By understanding the patients´ experiences the nurse can formulate the care to make it easier for the patient. Aim: The aim of the literature review was to illuminate elderly patients´ experiences of delirium in connection with a hospital visit.

This Masters' thesis is a study on the information behaviour of hospital personnel at the University Hospital in Lund, Sweden. A questionnaire was distributed to 301 nurses and physicians working in the intensive care unit, the population was reduced to 244 persons. The response rate was 52%, i.e. 125 questionnaires were completed. Nurses constituted 68 % of the population.

Studies show that older is an increased proportion patients in hospital and that it is a patient group that can be difficult to communicate with because of older patients' often multiple disease picture. Relatives to the patient can be help for caregivers in the communication and the care of the patient. The aim with the study was to examine relative's experience of participation, meeting/support, accessibility and information on a geriatrics rehabilitation care unit. The study had a descriptive design and was designed as a questionnaire study. It was implemented on one geriatrics rehabilitation care unit on the countryside in central Sweden, belong to Uppsala University hospital.

ABSTRACTProblems with high turnover in nursing has escalated during the last years both nationally and internationally. according to previous research job satisfaction and quality of care was connected to specific features of the hospital work environement. It was also recommended by Coomber & Barriball (2007) to analyze job satisfaction at ward level with qualitative interviews to get a deeper and more extended insight in which specific features are of importance.The aim of this study was to investigate and describe the experiance of job satisfaction in relation to quality of care from nurses perspective. This study was carried out at an orthopaedic ward with primary nursing as organizational context of care.Two focus group interviews were conducted, they comprised nine nurses with at least two years experiance of primary nursing. A qualitative content analysis with inductive approach was used in order to develop more detailed insights of potential factors influencing work satisfaction at ward level.

Upplevelser av kommunikationsproblem efter en stroke : En analys av självbiografier

Every year about 30000 people have a stroke. It is caused by either a bleeding or a blood clot, and depending on where the damage is, the patient can get different types of difficulties following after the stroke. About 40 % of all stroke patients sufferfrom some kind of speech and/or communication difficulty, like aphasia or dysarthria. As hospital staff it´s important to know how to communicate with these people, to get a deeper understanding regards their needs.Aim:The aim of this study is to out of a patient's perspectivedescribeexperiences ofcommunication difficulties that can follow after a stroke.Method:A content analysis with a qualitative approach was used to analyse five autobiographies.Results:The result showed in what way stroke patients experienced their communication difficulties, and how they felt about the hospital staffs treatment related to their communication difficulties. During the analysis two major themes emerged: suffering and wellbeing.

The Swedish mental hospitals existed for about one hundred years, the period around 1880-1980. The research is intended to be from a local setting perspective to study the local approach used to meet requirements from Furunäsets mental hospital principals 1960 - 1972, the effect this had on the care and treatment of mentally ill patients and how Swedish society's changing approach to mental health influenced the process. The results of the research showed that Furunäset searched for ways to meet the principals' requirements which often went against the patients? personal best. Att arbeta smartare en fallstudie om teorier och modeller inom Lean kan tillämpas i akutsjukvården

The emergency medical care in Sweden has been under persistent debate, due to factors such as long waiting hours, inefficient patient flow, and understaffed departments. In an effort to meet the critique and with a willingness to improve the problems, an increasing amount of emergency clinics are adopting theories and models within Lean. Positive outcomes are evident, there has been some criticism directed towards Lean. Among these is the argument that in order for Lean to be successful it needs to be used within standardized and foreseeable processes, and that it therefore only is applicable to those kind of processes found within industrial production. This prompted the question of whether Lean was a suitable strategy within emergency medical care, considering the large variation of patients and non-foreseeable processes that in fact share little resemblance to that of industrial production.

Införande av IT-system i slutenvården : En studie vid Lasarettet i Enköping

As the ability to computerize processes and tasks increase, the proliferation of complex systems in our society continues. How these systems are deployed in the organizations where they are meant to be utilized has been shown to have a large impact on how well they function.In January of 2011 the Enköping hospital initiated the deployment of an IT-support for drug management, a module in the general health care system Cambio COSMIC. Other health care providers in the county, and in the rest of Sweden, have had mixed feelings toward the module. Some have outright refused the module, claiming that it is detrimental to patient safety. One of the arguments for deploying the module in Enköping, however, was that it was needed to improve patient safety.The Enköping hospital is considered to be a well organized institution with good leadership and earlier good experiences regarding IT-systems.
