Personalens följsamhet till riktlinjer avseende glukoskontroll postoperativt efter Coronary Artery Bypass Graft (CABG)

 The aim of the study was auditing medical records examine postoperative blood glucose levels after undergoing CABG surgery. Furthermore the aim was to determine if the ward staff abides the local guidelines frame of reference concerning each ward, regarding blood glucose measurements and blood glucose levels. The study included 70 patients undergoing CABG surgery at the cardiothoracic surgery, Uppsala University Hospital. The study was conducted by retrospective medical record auditing. Studied factors were postoperative blood glucose levels, number of registered blood glucose measurements, a current diagnosis of DM and preoperative HbA1c.

The swedish law against female genital mutilation (FGM) prohibits procedures that removes parts of the female genitalia and thereby makes permanent changes in the body. The problem with the wording of the law is that it may also apply to the western phenomenon cosmetic genital surgery. This paper therefore examines if cosmetic genital surgery should be covered by the swedish law against FGM, by making a critical comparing analysis. The paper begins with comparing FGM with cosmetic genital surgery to prove that there are more similarities than differences between the procedures. By using the theoretical perspectives universalism and postcolonialism, the paper then examines why, particularly in the West, there is a different approach to FGM than to cosmetic genital surgery, regardless of the similarities of the procedures.

Dagkirurgiska patienters upplevelser av postoperativ smärta vid ortopedi-och bukkirurgi med beaktande av kön och ålder

The aim of this study was to elucidate day surgery patients´ subjective experiences of postoperative pain after orthopedic and abdominal surgery and if there was any difference in the experience according to sex and age.Selection was not random and the study included 87 patients. Data were collected from patient questionnaires. VAS method was applied in the questionnaire to measure patients´ pain.The outcomes of the study show that there was no significant difference between women´s and men´s experiences of pain during days 1-7.It was found that patients undergoing orthopedic surgery had significantly more pain on day 7 compared to those patients who underwent abdominal surgery.There was a significant negative correlation between age and perceived pain on day 7.Patients in day surgery group had significantly less pain on day 7 compared with day 1.The patients who have undergone orthopedic surgery and younger patients had more pain on day 7 while the patients in day surgery group had a pain level decreased gradually and on day 7 was the lowest.It appears that pain relief on day 7 of younger patients and patients who have undergone orthopedic surgery is an area that can be improved..

I denna studie undersöktes hur 291 individer som genomgått en Gastric Bypass för mer än två år sedan skattade sin psykosociala hälsa. Syftet med studien var att undersöka samband mellan den psykosociala hälsan och hur länge respondenterna hade varit opererade, eventuell viktuppgång samt hur länge i livet respondenterna haft obesitas. Mätinstrumenten som användes var Satisfaction With Life Scale (livstillfredsställelse), Social Physique Anxiety Scale (social kroppsångest) samt Rosenberg Self- Esteem Scale (självkänsla). Resultaten visade att ju längre tid det var sedan patienterna opererades samt ju mer patienterna ökade i vikt, desto lägre skattades den psykosociala hälsan. Vidare skattade de patienter som varit obesa hela livet, högre social kroppsångest jämfört med patienter som rapporterade att de haft perioder i livet som normalviktiga..

The purpose of the study was to review the current veterinary literature regarding gastric dilatation and volvulus (GDV) considering pathophysiology and treatment, and to evaluate the operative technique (incisional gastropexy) used during the years 2005 and 2006 at the University Animal Hospital (Universitetsdjursjukhuset), SLU, Uppsala, Sweden. GDV has a complicated pathophysiology which foremost effects the circulatory system. Progress is often acute and it is not unusual that patients are affected with shock. The initial treatment targets to alleviate the shock by fluid therapy and decompression of the stomach. There is a high risk of recurrence in the long term perspective unless the stomach is surgically fixated by gastropexy. Patientrelaterade riskfaktorer för postoperativ sårinfektion efter Coronary Artery Bypass Graft - CABG

Introduction Postoperative wound infections leads to increased patient suffering and cost society money because it leads to longer hospital stays. Previous research shows that patient related risk factors such as smoking, diabetes, Chronic Obstructive Pulmonary Disease (COPD), advanced age (>65 years) a low preoperative hemoglobin level (<140g/L) and a high BMI (> 30) increases the risk of suffering a postoperative wound infection. Aim To investigate whether patients that underwent surgery with Coronary artery Bypass Graft (CABG) in a university hospital in central Sweden in 2013 and had any of these risk factors more often suffer from post-operative wound infection.MethodThe study has a descriptive design and is based on an ongoing infection registration area of operations. The study included 148 patients out of 255 (58%) undergoing CABG in 2013. Possible risk factors were collected through medical record review.Results There was no significant correlation between surgical wound infection and patient-related risk factors such as smoking (not quit smoking four weeks before surgery), diabetes, COPD, advanced age (>65 years), low preoperative hemoglobin (<140g/L) or a high BMI (>30) and suffered a postoperative wound infection.Conclusion More research and a larger sample is needed in the field to produce a reliable result could be presented..

Kartläggning av preoperativa fasterutiner på tre vårdavdelningar

The aim of this study was to investigate for how long patients fast before thoracic surgery, plastic surgery and ear, nose and throat surgery, at the University hospital of  Uppsala, and what knowledge patients have about fasting and how they get affected by it. 30 patients were interviewed, 10 patients at each ward, according to a questionnaire. Data processing was performed with SPSS. There was no difference in fasting time for solids between the wards but the mean fasting time was 14 hours. In the thoracic ward patients fasted from liquids considerably longer than in the other wards.

Background: Anxiety with children before surgery is well known. From the 1950s there are various assessment instruments. Despite that most day surgery units offer preparation for children and parents, there are many children who exhibit a strong anxiety the day of surgery, which can lead to postoperative behavioral changes. Today the nurse estimates the anxiety of a child on personal experience and knowledge, forming the basis for estimation whether the child is in need of sedation or not. The purpose: of this study was to evaluate instruments that the nurse can use in measuring the degree of anxiety of children before fast clinical settings, such as day surgery.

Alkoholproblem före & efter bariatrisk kirurgi

Inledning: Andelen personer som lider av obesitas ökar markant i hela världen och för att återfå sin hälsa är så kallad fetmabehandlande, även kallad bariatrisk kirurgi, den sista utvägen för många. En operation som visar betydligt bättre resultat än medicinsk behandling även på lång sikt vad gäller vikt, effekt på diabetes typ 2, hypertoni mm. Men flera studier tyder på ökad risk för alkoholmissbruk, i alla fall efter Gastric Bypass (1, 2). Årligen opereras mellan 7000 och 8000 personer bara i Sverige och kunskapen kring kort- och långtidseffekterna av kirurgin ökar allt mer (3). För att resultatet av operationen ska förbli långsiktigt positivt krävs att patienterna varaktigt förändrar sin livsstil men tydligen bör de även uppmärksammas på att till exempel alkoholupptaget förändras och därmed kan öka risken för missbruk.

Postoperativt välmående efter överviktskirurgi

Introduktion: Depression, ångest, stress och ätstörningsproblematik har visat sig ha en hög prevalens hos personer som lider av obesitas. Syfte: Att undersöka om och i så fall hur ångest och depression mätt i HADS förändras i en grupp som genomgått överviktskirurgi. Av intresse var också att undersöka om viktnedgång samt om preoperativa symtom på stress, hetsätning och känslomässigt ätande predicerar utfallet av symtom på ångest och depression postoperativt. Metod: 30 patienter som genomgått överviktskirurgi på Akademiska sjukhuset i Uppsala under våren 2011 inkluderades i studien. Dessa patienter hade innan genomgången operation och vid en uppföljning sex månader postoperativt besvarat olika screeningformulär avseende psykiskt välmående, ätstörningar och stress.

Patienters skattning av hindrande smärta och self-efficacy före och efter diskbråcksoperation

Objectives: The purpose of the study was to examine if a pre- and post-operative difference could be seen in self-reported pain disability and self-efficacy among patients undergoing surgery as treatment for spinal disc herniation in order to improve process of rehabilitation.Method: A quantitative and descriptive design was used. 10 patients awaiting surgical treatment for spinal disc herniation were included at Akademiska sjukhuset (The Academic Hospital) in Uppsala. Participation was voluntary and the selection was made by convenience. Data was collected using The Pain Disability Index and Self-Efficacy Scale by which the patient would estimate how hindered they felt because of their pain in everyday activities and their self-efficacy to perform everyday activities. This was done the day before surgery and two weeks after surgery.

Sjuksköterskor har ansvar för att barn är behandlade i överensstämmelse med barnens rättigheter och att interagera med dem. I bakgrunden beskriver en studie att barn upplever att sjuksköterskor inte lyssnar på dem. Barn är sårbara eftersom de är underlägsna både sina föräldrar och sjuksköterskorna beroende på deras både åldersmässiga och kunskapsmässiga övertag. Det är därför viktigt för sjuksköterskor att ge barn möjligheten att delta i beslutsfattande processer, vilket kan få dem att känna sig inkluderade, respekterade och lyssnade på som individer. På så sätt kan sjuksköterskor kommunicera trygghet.
